Alert: Inaccurate drop rate, tested.
The wiki says that the totem piece drops into the inventory, or the ground if the inventory is full. However, it actually drops into the totem bag if your inventory is full.

[Updated drop chance: 1/528, tested by our maxed rs3 team, it has been dropping at around 500-550 kills per totem piece, with or without the Luck of the Dwarves.]

If you lose the piece, you can also reclaim it from the totem bag. If you lost your totem bag, you can reclaim it from an archaeologist at the Anachronia base camp.
....go north of Brutish Dinosaurs, place it. Charge the totem in the minigame! =]
Update: Reviewer #2 got the second drop at 1021 kills. He equipped the Luck of the Dwarves at 700 kills.

Killing dinosaurs on slayer task (Lv. 99) will double the loot drop
They can also be killed without slayer task.
Brutish Dinosaurs: Inquisitor staff weakness, recommend deflect from Melee
Both spots are able to block the path of the dinosaurs so that they can't reach you to retaliate, but occasionally the dinosaurs may back away from you, which pulls you out of your safe spot.
Sometimes the dinosaur is aggro'ed from a close angle and it may still reach you for its counterattack.
This aggro angle problem occurs more often at the Western safe spot.

For the part of the secrets of the asylum if you are finding the items to make the cures be careful with eating the item because you can't actually find a new one and you will have to buy or find it again outside of the asylum!
